  • Highmark (subject to policy)

Additional Fees: 

 

There is an additional $20 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Pre-Employment

  • Employment

  • Life Insurance.

  • Family Medical Leave
